FAQ’s

  • MindMotionPT does not accept insurance as a form of payment. Patients may be able to submit out-of-network claims to insurance, depending on insurance company, plan, and treatment. MindMotionPT will provide an invoice (‘superbill’) that can be submitted to your insurance with all of the necessary information (license numbers, tax ID, business name, etc.) as well as your specific diagnosis and billing codes. You may choose to submit bills after each visit or any other frequency, typically up to one year following the provided treatment visit. MindMotionPT cannot guarantee any reimbursement of self-submitted out-of-network claims.

  • Weekdays 9AM – 5PM. Visits outside of regular business days may be made however are not guaranteed on a weekly basis. Holiday hours vary. You will be informed ahead of time if a visit needs to be rescheduled.

  • No, MindMotion has no relationship with Medicare. As such, if you have a Medicare Insurance Plan, you may only seek services through a direct-pay, direct access practice like MindMotion PT that are not covered by medicare such as prevention, wellness, and fitness. MindMotion PT offers strength and conditioning privately that may be bought in conjunction with a Medicare insurance plan. This cannot be used as a substitute if you are seeking physical therapy.

  • No. California’s direct access method allows patients to seek care when they need it without going through their doctor for a prescription for physical therapy. A prescription is only required after the 12th visit or 45 days after evaluation.  According to treatment data, 90 percent of patients will not require services beyond 45 days or 12 visits.

  • MindMotion Physical Therapy accepts all major credit card and debit card transactions via point of sale through Square. Cash or check preferred. Other forms of payment available such as Venmo, PayPal, or Zelle. Receipt will be emailed with all payment types. Payment of package required in full at first appointment. No payment plans or pay-per-session are offered at this time. Bogdan Katkov will not be held accountable for funds distributed to incorrect recipients.

  • Of course! My rates for Physical Therapy and Strength Training are largely tied to the earnings potential of a Home Health Physical Therapist in San Jose, factoring in business expenses. Charging less than what I would earn in that role would render it financially impractical for me to continue this business. Based on Medicare reimbursements, home health PT pay-per-visit rates and cash-based PT clinic rates in the area, finding private in-home physical therapy services in San Jose for less than $200 per visit is exceedingly rare.
